About This Website
MMJ Card Cincinnati (medicalmarijuanacardcincinnati.com) provides informational resources, educational materials, and scheduling coordination to assist Ohio residents in connecting with independent, licensed Ohio physicians for medical cannabis evaluations.
MMJ Card Cincinnati is not a healthcare clinic, does not provide medical services, does not practice medicine, is not a licensed dispensary, and is not an agency of the State of Ohio. By accessing or using this website, you agree to comply with and be bound by these Terms & Conditions.
Medical Cannabis Evaluations
Medical cannabis consultations and evaluations coordinated through this website are conducted exclusively by independent, licensed Ohio physicians who hold an active Certificate to Recommend (CTR) issued by the State Medical Board of Ohio.
Recommending physicians practice independently and make all clinical assessments based entirely on their own professional medical judgment. Scheduling an appointment, submitting an inquiry, or completing an informational form does not guarantee that a physician will issue a medical cannabis recommendation. A physician may determine, based on clinical evaluation and medical history, that a patient does not meet the statutory criteria for an approved qualifying medical condition under Ohio law or that medical cannabis is clinically contraindicated.
Physician-Patient Relationship
Browsing this website, reading published educational materials, submitting an online inquiry, or completing an educational self-check or eligibility tool does not create or establish a doctor-patient or physician-patient relationship between you and MMJ Card Cincinnati or any healthcare practitioner.
A bona fide physician-patient relationship is established exclusively when an eligible patient participates directly in a clinical telehealth consultation with an independent licensed Ohio physician following formal intake, identity verification, and informed medical consent.
Patient Responsibilities
Patients and prospective patients are responsible for providing truthful, complete, and accurate information during appointment scheduling, pre-intake questionnaires, and clinical consultations.
You agree not to submit false, misleading, or fraudulent identity documentation, medical records, or symptom descriptions. You are also responsible for consulting with your primary healthcare provider regarding your ongoing medical care, current medications, and general health regimen.
Patients Under 18
Under Ohio law, prospective patients under the age of 18 are not categorically excluded from medical cannabis evaluations, but their participation is subject to specific statutory and regulatory requirements.
An evaluation for a patient under 18 requires the informed consent and participation of a parent, legal guardian, or designated legal representative. In addition, Ohio law requires an approved adult caregiver to be registered in the state registry on behalf of the minor patient. Patients under 18 cannot independently purchase medical cannabis products from Ohio dispensaries.
Ohio Patient Registration
A physician evaluation is a clinical assessment separate from state patient registration. If an independent physician issues an Ohio medical cannabis recommendation, that recommendation is submitted to the Ohio Patient & Caregiver Registry.
Patient registration, caregiver registration, and digital card activation are administered exclusively by the Ohio Division of Cannabis Control (DCC). MMJ Card Cincinnati does not manage the state registry, does not issue state medical marijuana cards, does not approve registry applications, and does not control state processing times, registry activation, or DCC administrative decisions.
Scheduling and Third-Party Services
When you select an appointment or scheduling option on MMJ Card Cincinnati, you may be directed to an external patient scheduling or telehealth platform.
Our current scheduling process uses Heally. Heally is an independent third party; MMJ Card Cincinnati does not own, operate, or control Heally, and Heally's terms, privacy practices, and payment systems are separate from these Terms & Conditions. When using Heally, you are subject to Heally's terms of service, privacy policy, payment terms, and telehealth informed consent disclosures. MMJ Card Cincinnati does not control all data, functionality, or technical performance within the third-party platform.
Fees and Payments
Fees displayed or collected in connection with physician consultations cover the evaluating physician's professional clinical time, medical chart review, and diagnostic evaluation service.
Payment of an evaluation fee does not guarantee that a physician will issue a medical cannabis recommendation, nor does it guarantee patient registration or state approval. Fees paid directly to third parties, external platforms, or state administrative bodies are subject to their respective terms. Specific cancellation, rescheduling, and refund rules are governed by the terms disclosed during booking and our approved Refund Policy.
Medical Cannabis Purchases
MMJ Card Cincinnati does not sell, distribute, dispense, process, or deliver cannabis or medical marijuana products.
A physician recommendation and an active digital registry card permit an Ohio patient or registered caregiver to purchase medical cannabis exclusively from licensed, state-authorized dispensaries operating under the oversight of the Ohio Division of Cannabis Control. Prices, product availability, taxes, and dispensary sales policies are set independently by individual licensed dispensaries and are not controlled by MMJ Card Cincinnati.
